Regulated Utility Account:
Copy of all pages of the most recent or current utility bill showing at least 22 days of service.
- Households with a disconnection emergency must include a copy of the 15-day or 48-hour, or shut-off notice in addition to the regular bill.
Sub-metered Accounts:
Copy of most recent rent receipt showing energy costs
Included in Rent:
Certification Letter completed and signed by the Landlord. The letter must specify the total amount of rent paid toward energy costs each month.
Wood/Propane/Oil:
Copy of most recent invoice showing amounts delivered and price per gallon or unit or total cost of delivery.
